Three steps to ship your samples
Pack samples with a completed chain-of-custody form before you ship.
1 · Enter your code & create the label
Enter your access code above to open the FedEx label tool, add your package details, and generate a prepaid return label addressed to our Union, NJ intake.
2 · Print & attach
Print the label, seal your package with the chain-of-custody inside, and attach the label to the outside.
3 · Drop it off
Hand it to any FedEx location or scheduled pickup. Results follow our standard same-day, 24 and 48-hour turnaround once received.
