Your PDF should be downloading now, and your email client should have opened with a copy addressed to our intake team. Print the PDF and ship it with your cooler.
How this works
A real-time tracking portal is in development. In the meantime, this form gives you the two things you actually need today: a clean printable CoC, and a digital copy in our intake inbox before the cooler arrives.
1. Fill the form
Add as many sample rows as you need. The PDF will use whatever you enter exactly.
2. Click "Generate"
The form downloads a print-ready PDF and opens your email client with the submission already drafted to intake@irislaboratories.com.
3. Send the email, print the PDF
Hit Send in your email client. Print the PDF, slip it into the cooler with your samples, ship as usual.
4. We confirm receipt
When the cooler arrives, intake matches the physical CoC against the email and you'll get a "received" confirmation back.
